Integrated Disability Management

Horizon Behavioral Service’s One-on-One Disability Care Program is a customized service, designed to reduce the duration of your employee’s long-term psychiatric disability, and help your employee return to work.

As a core EAP tool, our One-on-One program works across four key areas: training, outreach, education and coaching. Our goal is to increase personal functioning and encourage recovery. Key components of Horizon Behavioral Service’s One-on-One program include:

Disability Vendor Training – We work through the employer to train the Disability Vendor (intake and claims specialists) on how to recognize cases where the Employee Assistance Program can play a valuable role. Training includes an introduction to Horizon Behavioral Service’s EAP protocols and guidelines, and successful transfer procedures to effectively integrate disability care.

Proactive Outreach for High Risk Cases – We partner with the employer and their Disability Vendor to identify high-risk cases that could benefit from proactive EAP outreach based on diagnosis, acuity of care, and established triggers. If the member chooses to participate, we will perform outreach and create a personal recovery plan.

Educational Packers for Claimants – For cases that are lower risk or less intense, we educate around issues that contribute to disability such as stress, depression and behavior change tools.

Life Coaching by a Master’s-level Clinical Specialist – Working in partnership with a Disability Vendor, we will identify employees who may be subject to an extended leave due to those external factors such as an aging, ill parent, a sick child, or disease co-morbidity. Our WorkLife and Wellness programs play an important role in finding the resources to help your employee resolve extenuating factors and reducing risk of other illnesses.

Return-to-Work Integration and Coaching – To aid employees, we may recommend support groups that provide professional assistance in navigating issues and integration – or provide regular telephonic follow-up and coaching.
